Parents or carers requesting the administration of medication should read our Supporting Pupils with Medical Conditions Policy which can be found on our Policies page.

Parents and carers must then complete the following form. 

The school is unable to give children any medication unless this form is completed and the SENCo or a school leader has confirmed that school staff have agreed to administer the medication.
